This form is to be completed by people who are proposing to start or continue in child-related employment and who have already applied for, or hold, a disability worker screening clearance issued in Queensland.

Use this form to apply for, renew an existing, or transfer from a volunteer blue card to a paid card.

• scan and upload
• post, or
• delivering it in person.

Completing a paper form requires you either:

• verify your identity online and include your account number on the paper form
• provide a certified copy of your ID document.
