Volunteer to business transfer

This form is to be completed where a volunteer applicant/blue card holder is proposing to undertake child-related business and needs to transfer from a current volunteer 'V' card to a paid 'P' card. 'V' card must not be expired.

You can also do this using the online applicant portal.

You can contact us and request this form be sent to you, or you can download, complete and submit a form by:

• scan and upload
• post, or
• delivering it in person.

Completing a paper form requires you either:

• verify your identity online and include your account number on the paper form
• provide a certified copy of your ID document.
