This form is used to apply to the department to temporarily suspend the training contract for reasons such as seasonal employment, long term illness/injury or maternity/paternity arrangements.
NOTE: This form is NOT to be used by employers wanting to suspend their apprentice or trainee for any employment related matters such as serious misconduct, invoking disciplinary measures or other workplace issues.
