Access donor conception information application For a donor (Form 23B)
On 16 September 2026, new laws started that makes information available about people involved in donor conception procedures that took place in Queensland and resulted in a child being born.
Find out how to apply, visit www.qld.gov.au/rbdmdonorregister.
Use this form if you are unable to apply through the online portal to:
- access the donor conception information register
- set your consent preferences
- add details about yourself and medical information to the register
- set your notification preferences.
