Early pregnancy loss recognition certificate application
Use this application form is to help parents whose baby was born stillborn or miscarried before 20 weeks gestation or, if weeks are unknown, weighed less than 400g. We are not able to register the birth, but families have the right to commemorate their loss with a free recognition certificate.
