A property agent and their client must fill out this form in order to have a valid appointment. Agents cannot represent a client unless they are properly appointed.
A property agent must fill out this form if they or a close associate have a personal or business interest in the purchase of a client's property. The agent will need to provide the completed form to the client before the sale.
Property agents and property developers must fill out this form to tell a prospective buyer if they have any relevant interests in the sale (including third-party interests).
