Application to cancel a negative notice

This form is for people who hold a negative notice, and can only be submitted 2 years after the negative notice was issued.

You can contact us and request this form be sent to you, or you can download, complete and submit a form by:

• scan and upload
• post, or
• delivering it in person.

Completing a paper form requires you either:

• verify your identity online and include your account number on the paper form
• provide a certified copy of your ID document.
