Form 28 - Application for temporary authority

This form is to be completed if licensed premises is temporarily moving to a new location (e.g. if the current premises is being rebuilt, refurbished or is damaged by fire). You may also need to complete Form 23 ‘Application to alter, rebuild or change the premises’ if you are making structural changes to the current premises. The Office of Liquor and Gaming Regulation doesn’t accept credit card details by email. If you email them to us, we’ll immediately delete them and your payment won’t be processed.
