Personal probity (short) form category 2 gaming licensees (clubs)

Each executive officer or member of the Management Committee of a club must complete this form. An executive officer of a body corporate means a person who is concerned in, or takes part in, the management of the body, regardless of the person’s designation or whether or not the person is a director of the body. All committee members of a club are considered executive officers.
