Personal probity (short) form category 1 gaming licensees (hotels & taverns)

Each executive officer, shareholder or beneficiary (over the age of eighteen) under a trust must complete this form. An executive officer of a body corporate means a person who is concerned in, or takes part in, the management of the body, regardless of the person’s designation or whether or not the person is a director of the body corporate.
