Use one of these forms to update your bank details with the State Penalties Enforcement Registry (SPER) so that we can pay your refund by electronic funds transfer.

You can complete an online version of the SPER EFT request form without the need to download a printable copy.

When completing the form, you'll need your SPER client number (also referred to as a party ID).
