Related Victim - Financial Assistance Application Form - Victim Assist Queensland

This form is for a close family member or dependant of a person who has died as a direct result of an act of violence that has happened in Queensland (e.g. homicide or dangerous operation of a motor vehicle causing death).

Under the Victims of Crime Assistance Act 2009, a close family member includes children, siblings, parents, spouse or partner of the victim. If the victim was of Aboriginal, Torres Strait Islander, or Aboriginal and Torres Strait Islander culture, other family members who have a relationship like that of a child, sibling, or parent may also be considered a close family member.
