Witness - Financial Assistance Application Form - Victim Assist Queensland

An application form for a witness to an act of violence occurring in Queensland who wants to seek financial assistance.

Under the Victims of Crime Assistance Act 2009 this includes someone who has been psychologically or emotionally injured by seeing or hearing an act of violence.

An act of violence includes, but is not limited to, homicide, any form of assault, a sexual offence, domestic/family violence and a range of other personal offences).

Do not use this form if the violence was against yourself — you need to complete the Primary Victim - Financial Assistance Application Form.
